Matcha and Medications: Known Interactions and What to Watch For

Matcha can interact with several medications through multiple mechanisms: caffeine affects CYP1A2 enzyme activity (altering drug metabolism), EGCG inhibits certain drug transporters, vitamin K in matcha can affect anticoagulants, and tannins can reduce iron absorption. The most clinically significant interactions involve blood thinners (warfarin), MAOIs, stimulant medications, and certain antibiotics. Why matcha can interact with medications
Matcha contains several bioactive compounds that can affect how medications are absorbed, metabolized, or eliminated:
1. Caffeine: affects the CYP1A2 liver enzyme, which metabolizes many medications 2. EGCG (epigallocatechin gallate): inhibits certain drug transporters (P-glycoprotein, OATP1B1) 3. Vitamin K: present in matcha; relevant for anticoagulant medications 4. Tannins: can bind to certain medications and reduce absorption 5. L-theanine: mild effects on neurotransmitter systems
The clinical significance of these interactions varies widely. Some are well-documented and clinically important; others are theoretical or observed only at very high doses. This article covers the most relevant interactions based on available evidence.

Blood thinners (anticoagulants)
Warfarin (Coumadin) is the most clinically significant interaction with matcha.
Matcha contains vitamin K, which is a cofactor in the clotting cascade. Warfarin works by inhibiting vitamin K-dependent clotting factors. Consistent vitamin K intake is important for stable warfarin dosing -- sudden changes in vitamin K intake can cause INR (International Normalized Ratio) to fluctuate, increasing bleeding or clotting risk.
The key principle: consistency matters more than avoidance. If you regularly consume matcha and your warfarin dose is calibrated to your typical vitamin K intake, maintaining that consistent intake is more important than eliminating matcha entirely.
However, if you are starting or stopping matcha consumption, or significantly changing your intake, inform your prescribing physician so your INR can be monitored and your dose adjusted if needed.
EGCG in matcha may also have mild antiplatelet effects, which could theoretically add to anticoagulant effects. The clinical significance of this is not well-established.

MAOIs (monoamine oxidase inhibitors)
MAOIs are a class of antidepressants (phenelzine, tranylcypromine, selegiline) that inhibit the enzyme monoamine oxidase. Caffeine interacts with MAOIs in two ways:
1. MAOIs inhibit CYP1A2, the enzyme that metabolizes caffeine. This can significantly extend caffeine's half-life, causing caffeine to accumulate to higher levels than expected. 2. The combination of caffeine and MAOIs may increase the risk of hypertensive episodes.
People taking MAOIs should discuss caffeine intake, including matcha, with their prescribing physician. Significant reduction or elimination of caffeine may be recommended.

Stimulant medications (ADHD medications)
Stimulant medications used for ADHD (amphetamines such as Adderall, methylphenidate such as Ritalin) have additive stimulant effects with caffeine. Combining matcha with stimulant medications can:
- Increase heart rate and blood pressure beyond what either substance causes alone - Worsen anxiety or jitteriness - Disrupt sleep more than either substance alone
The clinical significance depends on the dose of both the medication and the caffeine. At typical matcha doses (60 to 80mg caffeine), the interaction is generally mild for most people, but individual sensitivity varies.
If you take stimulant medications and notice increased heart rate, anxiety, or sleep disruption when consuming matcha, discuss this with your prescribing physician.

Antibiotics (fluoroquinolones)
Fluoroquinolone antibiotics (ciprofloxacin, levofloxacin, norfloxacin) inhibit CYP1A2, the enzyme that metabolizes caffeine. Taking fluoroquinolones while consuming matcha can significantly extend caffeine's half-life, causing caffeine to accumulate.
Practical implication: if you are prescribed a fluoroquinolone antibiotic, consider reducing or eliminating caffeine intake during the course of treatment to avoid caffeine accumulation and its associated effects (anxiety, insomnia, elevated heart rate).
This interaction is temporary -- it resolves when the antibiotic course is completed.

Iron supplements and iron-rich foods
Tannins in matcha (and other green teas) can bind to non-heme iron (the form found in plant foods and supplements) and reduce its absorption. This is relevant for people taking iron supplements or who rely on plant-based iron sources.
To minimize this interaction: - Take iron supplements at least 1 to 2 hours before or after consuming matcha - Avoid drinking matcha with iron-rich meals if iron deficiency is a concern - Consuming vitamin C with iron-rich foods can partially counteract tannin inhibition
This interaction is not unique to matcha -- it applies to all green teas and many other tannin-containing foods (coffee, black tea, red wine).

Medications metabolized by CYP1A2
Caffeine is metabolized by the CYP1A2 liver enzyme, and caffeine can inhibit or compete with other CYP1A2 substrates. Medications metabolized by CYP1A2 include:
- Clozapine (antipsychotic) - Olanzapine (antipsychotic) - Theophylline (bronchodilator) - Tizanidine (muscle relaxant) - Some antidepressants (fluvoxamine, duloxetine)
